PARQ and Health History Digital Agreement Process
This document outlines the process for handling PARQ (Physical Activity Readiness Questionnaire) and health history through a digital agreement signing system. The client is guided through signing an agreement, completing the PARQ, and storing their responses securely.
Step 1
During the agreement signing process, the PARQ and health history questions are included as part of the sign-up. When a client accesses the sign-up link, they will find PARQ questions embedded within the agreement. Upon proceeding to the next step, clients will see the agreement, and within the second section of the agreement, they will find the PARQ questions, which can be answered with a simple yes or no.

Step 2
The client can quickly respond to each PARQ question directly within the digital form. After completing the agreement process, they digitally sign the document to finalize the procedure.

Step 3
Once the client has completed the PARQ and signed the waiver, the entire document, along with their PARQ responses, is automatically stored in the client profile under attachments. By accessing the software and navigating to the client profile, you can view the signed agreement. Within this agreement, in the second section, you will find the PARQ responses, all accompanied by a digital signature.

Step 4
Additionally, a copy of the signed agreement, including the PARQ answers, is sent to the client via email or WhatsApp. This system ensures that the PARQ is captured, stored, and a copy is provided to the client seamlessly.
